What the job is
Faircape Health Institute is looking for a Rehabilitation Coordinator to join their team in Tokai, Western Cape. This company specializes in sub-acute rehabilitation and long-term care, focusing on helping patients recover using the best clinical practices. The Rehabilitation Coordinator will play a key role in making sure patients have a smooth journey from admission to discharge, with a focus on achieving the best possible recovery outcomes.
What you will do
You will work closely with a team of healthcare professionals to align patient goals and support their recovery. Your main tasks will include coordinating discharge plans tailored to each patient’s medical, functional, and social needs. You will communicate with patients and their families to keep them informed about progress, goals, and discharge expectations. You will also arrange discharge logistics such as home care, transport, and follow-up services. Additionally, you will facilitate family meetings, provide emotional support, and ensure all discharge documentation is accurate and completed on time.
What you need
You must have a Bachelor’s Degree in Social Work, Occupational Therapy, or Physiotherapy. Experience in discharge planning, case management, or rehabilitation environments is preferred, especially in acute or sub-acute settings. Strong communication, organizational, and interpersonal skills are essential. You should be empathetic, patient-focused, and able to work well in a fast-paced, multidisciplinary team. A clear health, credit, and criminal record are required, along with positive recommendation letters.
